          What Is Keratoconus?
         &#xD;

          Keratoconus (often abbreviated KC) is a progressive eye condition in which the cornea — the clear, dome-shaped front surface of the eye — gradually thins and loses its round shape, bulging outward into an irregular cone.
         &#xD;

  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;

          Under normal circumstances, the cornea has a smooth, symmetrical curve that bends (refracts) light precisely onto the retina, producing clear vision. In keratoconus, the structural integrity of the corneal tissue breaks down over time, causing it to distort. As the shape becomes increasingly irregular, light scatters as it enters the eye rather than focusing cleanly — resulting in blurry, distorted, and frequently shifting vision that glasses often cannot fully correct.
         &#xD;

          The exact cause of keratoconus isn't fully understood, but research has identified several contributing factors:

          Genetic Factors

          Eye Rubbing

          Chronic, vigorous eye rubbing is one of the strongest modifiable risk factors for both developing and accelerating keratoconus. Rubbing exerts mechanical stress on the corneal tissue and triggers the release of inflammatory enzymes that degrade the collagen fibers holding the cornea in shape. Patients with allergies — who often rub their eyes frequently — are at particular risk. If you have keratoconus, stopping eye rubbing is one of the most important things you can do.

          Keratoconus symptoms are often subtle at first and easy to attribute to ordinary nearsightedness or astigmatism. As the condition progresses, they become harder to ignore. Typical signs include:

          A standard eye exam — even a thorough one — may not catch keratoconus, particularly in its early stages. Accurate diagnosis requires corneal topography or tomography: imaging technology that maps the shape, curvature, and thickness of the entire corneal surface in fine detail.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          Stages of Keratoconus

          Keratoconus is typically classified into four stages based on corneal curvature and thickness measurements:
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Stage 1 — Mild
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Early corneal steepening and thinning, often with only subtle visual symptoms. Glasses or soft contact lenses may still provide reasonable correction. This is the ideal stage for corneal cross-linking if progression is confirmed.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Stage 2 — Moderate
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;

          More pronounced corneal irregularity. Standard soft contact lenses typically no longer provide adequate vision. Specialty contact lenses — scleral or RGP designs — become necessary to vault over the irregular corneal surface and provide a smooth optical front surface.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          Stage 3 — Advanced
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;

          Significant corneal thinning and steep, irregular curvature. Vision correction requires specialty lenses. Corneal cross-linking is still possible at this stage in many cases, though the benefit of halting progression is weighed against the already significant structural change.

          Extreme thinning, very high corneal curvature, and in some cases corneal scarring from chronic stress on the tissue (called hydrops). Contact lens tolerance may be lost, and corneal transplant surgery is often the appropriate next step.

          The right treatment depends on the stage of keratoconus and whether it is still progressing. Here is a clear overview of what each option does — and when it applies.

          Specialty Contact Lenses
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;

          For most patients with keratoconus, specialty contact lenses are the primary tool for achieving functional vision. Unlike soft lenses, which conform to the cornea's irregular shape, specialty lenses vault over or fit precisely to the irregular surface — providing a smooth, regular optical front surface for light to pass through.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;ul&gt;&#xD;
    &lt;li&gt;&#xD;
      &lt;strong&gt;&#xD;
        
           Scleral lenses
          &#xD;
      &lt;/strong&gt;&#xD;
      &lt;span&gt;&#xD;
        &lt;span&gt;&#xD;
          
            — large-diameter lenses that rest on the white of the eye (sclera) and completely vault the cornea, with a reservoir of saline solution between lens and cornea. Highly effective for moderate to advanced keratoconus. Excellent comfort. Our most commonly fitted specialty lens.
           &#xD;
        &lt;/span&gt;&#xD;
      &lt;/span&gt;&#xD;
    &lt;/li&gt;&#xD;
    &lt;li&gt;&#xD;
      &lt;strong&gt;&#xD;
        
           RGP (rigid gas-permeable) lenses
          &#xD;

            — smaller hard lenses that sit directly on the cornea. Very effective optically; some patients find them less comfortable than sclerals but they remain an excellent option for many.

            — a rigid center with a soft outer skirt. Combines the optics of a hard lens with improved comfort.

          Corneal cross-linking
         &#xD;
    &lt;/a&gt;&#xD;

      &lt;span&gt;&#xD;
        
           is the only FDA-approved treatment that stops keratoconus from getting worse. It does not reverse existing damage or reshape the cornea — it halts progression by strengthening the collagen bonds within the corneal tissue using riboflavin (vitamin B2) eye drops and controlled ultraviolet-A light.
          &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          CXL is most effective when performed early — before significant corneal thinning or distortion has occurred. This is why regular monitoring and early diagnosis matter so much. Most patients require no additional surgery after successful cross-linking and continue to manage their vision with specialty lenses.

The most common procedure for keratoconus today is DALK (deep anterior lamellar keratoplasty), which replaces the front layers of the cornea while preserving the patient's own endothelium — resulting in faster recovery and lower rejection risk than traditional full-thickness transplant (PKP).

          PRK is often the procedure of choice — or the only safe option — for patients who do not meet the criteria for LASIK. Common reasons to recommend PRK include:

  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          Thin or Irregular Corneas
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;

          Creating a LASIK flap requires a certain amount of corneal tissue. Patients with thinner-than-average corneas may not have enough residual tissue after flap creation and laser treatment to be safe LASIK candidates. PRK removes only the epithelium, preserving more of the underlying stroma.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Contact Sports or High-Risk Activities
         &#xD;
    &lt;/span&gt;&#xD;

  &lt;p&gt;&#xD;

          The LASIK flap never fully fuses back to the strength of the original corneal tissue. While flap dislodgement is rare with normal activity, it can occur with a direct blow to the eye. Patients who engage in boxing, martial arts, hockey, rugby, or other high-contact activities — as well as military personnel and first responders — are generally advised to choose PRK.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Dry Eye
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
          LASIK severs more corneal nerves during flap creation, which can worsen pre-existing dry eye and cause new dry eye symptoms in some patients. PRK, which operates on the corneal surface, tends to have a lower impact on the corneal nerve density and may be more appropriate for patients with mild to moderate dry eye.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;

          Large Pupils
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;

          In some patients with very large pupils, the treatment zone of a LASIK flap may not fully cover the optical area in low light, potentially causing halos or glare at night. PRK can sometimes allow for a slightly larger treatment zone in these cases.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          What About SMILE?
         &#xD;

          SMILE (Small Incision Lenticule Extraction) is a newer laser vision correction procedure that removes a small disc of corneal tissue through a tiny keyhole incision — no flap, no surface removal.           What Is Dry Eye Disease?

          Dry eye disease (also called dry eye syndrome or keratoconjunctivitis sicca) occurs when the eye's tear film is unstable or insufficient — either because the eye doesn't produce enough tears, or because the tears it does produce evaporate too quickly.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

          The tear film is a thin, multi-layered structure that covers the surface of the eye with every blink. It has three layers:
         &#xD;

    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;ul&gt;&#xD;

           The outer lipid (oily) layer
          &#xD;
      &lt;/strong&gt;&#xD;
      &lt;span&gt;&#xD;
        &lt;span&gt;&#xD;
          
            — secreted by the meibomian glands along the eyelid margins; this layer prevents tears from evaporating too quickly
           &#xD;
        &lt;/span&gt;&#xD;
      &lt;/span&gt;&#xD;
    &lt;/li&gt;&#xD;
    &lt;li&gt;&#xD;
      &lt;strong&gt;&#xD;
        
           The middle aqueous (watery) layer
          &#xD;
      &lt;/strong&gt;&#xD;
      &lt;span&gt;&#xD;
        &lt;span&gt;&#xD;
          
            — produced by the lacrimal glands; provides moisture and nutrients to the cornea
           &#xD;
        &lt;/span&gt;&#xD;

      &lt;strong&gt;&#xD;
        
           The inner mucin (mucus) layer
          &#xD;
      &lt;/strong&gt;&#xD;

            — secreted by goblet cells in the conjunctiva; helps the tear film adhere smoothly to the eye's surface
           &#xD;
        &lt;/span&gt;&#xD;

  &lt;/ul&gt;&#xD;
  &lt;p&gt;&#xD;

          When any of these layers is disrupted, the tear film breaks down between blinks — exposing the corneal surface to air and creating the symptoms of dry eye.
         &#xD;

    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          The Two Main Types of Dry Eye
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h3&gt;&#xD;
  &lt;h4&gt;&#xD;

          Aqueous-Deficient Dry Eye
         &#xD;
    &lt;/span&gt;&#xD;

          The eye simply doesn't produce enough watery tears. The lacrimal glands — tucked under the upper eyelid — are underproducing. This is less common than evaporative dry eye but tends to be more severe when it occurs. Autoimmune conditions like Sjögren's syndrome, rheumatoid arthritis, and lupus can cause or accelerate aqueous-deficient dry eye.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Evaporative Dry Eye (Meibomian Gland Dysfunction)
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Far more common — accounting for approximately 85% of dry eye cases. The problem here is not the volume of tears but their quality. The meibomian glands, which line the edges of both eyelids, produce the oily layer that prevents tears from evaporating. When these glands become blocked, inflamed, or dysfunctional (meibomian gland dysfunction, or MGD), the oily layer thins or disappears — and even an adequate volume of tears evaporates rapidly, leaving the corneal surface exposed.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
          MGD is often invisible to patients and many practitioners without specific examination techniques. It's one of the most underdiagnosed conditions in eye care.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h3&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Common Causes and Risk Factors
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h3&gt;&#xD;
  &lt;p&gt;&#xD;

          Dry eye disease has many contributing factors, and most patients have more than one at play:
         &#xD;
    &lt;/span&gt;&#xD;

  &lt;/p&gt;&#xD;

          Age

          Tear production naturally decreases with age. Dry eye is significantly more prevalent in adults over 50.

          Hormonal Changes

          Women are disproportionately affected by dry eye disease — particularly during and after menopause, during pregnancy, and when using oral contraceptives. Androgens play a role in meibomian gland function, and falling androgen levels contribute to MGD.

  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;

          Screen Use
         &#xD;
    &lt;/span&gt;&#xD;

          When we concentrate on screens, our blink rate drops by up to 60%. Incomplete blinks are also common — the eye closes partially without fully spreading the tear film. Modern digital device use is one of the primary drivers of dry eye prevalence, particularly in younger patients.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;

          Contact Lenses
         &#xD;
    &lt;/span&gt;&#xD;

          Contact lens wear disrupts the tear film and accelerates tear evaporation. Many patients who develop dry eye symptoms first notice them as contact lens intolerance — lenses become uncomfortable earlier in the day, or can no longer be worn for full workdays.
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;h4&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Medications
         &#xD;
    &lt;/span&gt;&#xD;
  &lt;/h4&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
          Antihistamines, decongestants, antidepressants, blood pressure medications, and hormonal therapies are among the most common medications that reduce tear production or alter tear composition. If you take any of these and are experiencing dry eye symptoms, it's worth discussing with both your eye doctor and prescribing physician.

           Change your clothes and shower after outdoor activities to remove pollen from your skin and hair.
