Medical vs. Vision Insurance

This is the question we hear most often from new patients — and it's an important one to understand before your visit, because the two types of insurance cover very different things.

Medical Insurance

Medical insurance — including Medicare, Aetna, Horizon BCBS, Cigna, and the other plans listed above — covers eye care that is medically necessary. This includes:

  • Diagnosis and management of eye diseases — glaucoma, diabetic retinopathy, macular degeneration, cataracts, keratoconus, dry eye disease, corneal conditions
  • Evaluation of symptoms — sudden vision changes, floaters, flashes, eye pain, redness, or any eye problem that prompts a visit
  • Surgical procedures — cataract surgery (standard IOL), corneal cross-linking, corneal transplants, glaucoma surgery, and most other procedures
  • Pre-operative and post-operative visits
  • Monitoring of chronic conditions — annual diabetic eye exams, glaucoma check-ups, retinal monitoring

If you are coming in for any medical reason — not just to update your glasses — medical insurance is what applies.

Vision Insurance

Vision insurance — such as VSP — covers routine eye care and hardware. This typically includes:

  • Annual routine vision exams (refraction — the glasses prescription test)
  • Allowance toward frames and prescription lenses
  • Contact lens fittings and a contact lens allowance
  • In some plans, a contribution toward specialty lenses

Vision insurance does not cover the medical component of your exam — only the routine vision assessment portion. If both types of coverage apply to your visit, we will bill each appropriately.

question and answer

If You Have Both

Many patients have both medical and vision insurance. If that's you, bring both cards. Our team will assess the purpose of your visit and bill each plan for the appropriate portion — maximizing your benefits and minimizing your out-of-pocket cost. You do not need to sort this out before your visit; our billing team handles it.

Medicare Patients

Medicare Part B covers medically necessary eye care — including the diagnosis and treatment of eye diseases, as well as cataract surgery with a standard monofocal IOL. Medicare does not cover routine vision exams for glasses or contact lens prescriptions, and does not cover the cost upgrade to a premium IOL (multifocal, extended depth-of-focus, or toric lens). If you are enrolled in a Medicare Advantage plan, coverage details vary by plan — bring your Medicare Advantage card and we'll verify your benefits.

AARP Medicare Supplement plans (Medigap) are accepted and can cover cost-sharing — such as deductibles and co-insurance — for services covered by Original Medicare Part B.

What Is — and Isn't — Typically Covered

Coverage varies by plan and by the medical reason for your visit. The best way to know what will be covered before you arrive is to call us — our team verifies benefits on your behalf.

  • Comprehensive dilated eye exam when medically indicated
  • Diagnosis and management of glaucoma, diabetic retinopathy, macular degeneration, cataracts
  • Cataract surgery with standard monofocal IOL
  • Corneal cross-linking (CXL) for keratoconus — subject to medical necessity criteria
  • Corneal transplant surgery
  • Glaucoma laser treatment (SLT)
  • Dry eye evaluation and medically necessary treatment
  • Medically necessary specialty contact lenses (e.g., scleral lenses for keratoconus) — subject to plan terms
  • Pre-operative and post-operative surgical visits.

Frequently Asked Questions

  • Does Affiliated Eye Surgeons accept Medicare?

Yes — Affiliated Eye Surgeons accepts Medicare Part B at all three of our Northern New Jersey locations. Medicare covers medically necessary eye care, including the diagnosis and treatment of eye diseases such as cataracts, glaucoma, diabetic retinopathy, macular degeneration, and corneal conditions, as well as cataract surgery with a standard monofocal IOL. Medicare does not cover routine vision exams for glasses prescriptions or the cost upgrade to a premium IOL. AARP Medicare Supplement plans are also accepted.

  • Does Affiliated Eye Surgeons accept Horizon Blue Cross Blue Shield?

Yes — Affiliated Eye Surgeons accepts Horizon Blue Cross Blue Shield and BC/BS OMNIA Tier 1 plans at all three locations. Coverage for specific services depends on the type of plan and the reason for your visit. To confirm that your specific Horizon plan is accepted and to understand your benefits before your appointment, call any of our offices directly: Morristown (973) 984-5005, West Orange (973) 736-3322, or Denville (973) 586-2188.

  • Does Affiliated Eye Surgeons accept Aetna?

Yes — Aetna is accepted at all three Affiliated Eye Surgeons locations in Morristown, West Orange, and Denville. If you have an Aetna medical plan, bring your insurance card to your appointment and our team will verify your benefits and apply your coverage. If you are unsure whether your specific Aetna plan is included, please call us before scheduling.

  • Does Affiliated Eye Surgeons accept VSP vision insurance?

Yes — Affiliated Eye Surgeons accepts VSP vision insurance at all three locations. VSP typically covers routine vision exams for glasses or contact lens prescription updates, plus a frames and lens allowance. VSP does not cover medical eye care — if your visit involves the diagnosis or management of an eye disease or medical symptom, that portion is billed to medical insurance. If you have both VSP and a medical plan, bring both cards and our team will bill each appropriately.

  • Does Affiliated Eye Surgeons accept Cigna?

Yes — Cigna is accepted at Affiliated Eye Surgeons. As with all insurance plans, coverage depends on your specific Cigna plan and the nature of your visit. Call any of our three offices to confirm your plan details before scheduling: Morristown (973) 984-5005, West Orange (973) 736-3322, or Denville (973) 586-2188.

  • Does Affiliated Eye Surgeons accept Medicaid?

No — Affiliated Eye Surgeons does not participate in any Medicaid plans. If you have Medicaid and would like to be seen, please call our office before scheduling to discuss out-of-pocket costs. Private pay patients are always welcome, and our team can provide fee information before your visit so you can make an informed decision.

  • Is LASIK covered by insurance?

LASIK and PRK laser vision correction are elective refractive procedures and are not covered by medical insurance or Medicare. Patients pay for LASIK and PRK out of pocket. FSA and HSA funds can be used for laser vision correction, and patient financing options may be available. The consultation to evaluate your candidacy for LASIK may be covered depending on your plan — call us to confirm.

  • Is cataract surgery covered by insurance?

Yes — cataract surgery is covered by Medicare and most major medical insurance plans when the cataract is causing clinically significant vision impairment. The surgical procedure and a standard monofocal intraocular lens are covered. If you choose a premium IOL — such as a multifocal, extended depth-of-focus (Symfony), or toric lens — the standard surgical fee is still covered, but the additional cost of the premium lens upgrade is not covered by insurance and is the patient's responsibility. Our team provides full cost information during your pre-operative consultation.

  • Can I use my FSA or HSA for eye care at Affiliated Eye Surgeons?

Yes — Flexible Spending Account (FSA) and Health Savings Account (HSA) funds are accepted at all three Affiliated Eye Surgeons locations. FSA and HSA can be used for a wide range of eye care services, including medical eye exams, prescription glasses and contact lenses, specialty contact lens fittings, LASIK or PRK surgery, and most medically necessary treatments. If you have FSA funds with a year-end deadline, contact us to schedule before the deadline — many eye care services qualify.
